We have released the July edition of our community mental health transformation newsletter.

What is the community mental health transformation?

The community mental health transformation is a national programme in the NHS Long Term Plan. This programme will ensure that the Trust works with primary care, local authorities, the voluntary sector and broader organisations to transform mental health care and support in the Black Country. The Trust aims to deliver care and support that is more accessible, flexible and tailored to suit an individual’s needs.

To help people understand this project, NHS England has created an animation:

We have created an animation to show how someone’s journey through services will change after the community mental health transformation:

In this edition of our newsletter, we focus on several key messages:

  • The theme of the month for July: Integrated Community Model – this shows how teams will work together to deliver the best care possible in a timely manner for people living with severe mental illness
  • Help rename our Community Teams – Share your creative ideas and suggestions on our Padlet
  • Date for your diary – Find out about Community Transformation Week in September
  • Download our email signature to help people find more information about how we are transforming community mental health services

The latest edition of our newsletter is available to read on our website.
