Walsall Together, of which Black Country Healthcare is a partner, has been selected as one of just 43 national sites to lead the first phase of the government’s National Neighbourhood Health Implementation Programme, aimed at transforming care, tackling health inequalities, and bringing support closer to home.
Working alongside NHS England, local authorities, GPs, voluntary and social sector partners and our communities, we’ll focus on supporting adults with multiple long-term conditions and moderate to severe frailty, building on our existing neighbourhood model to deliver truly integrated, proactive care.

We’ll also be part of an intensive national coaching and development programme, supporting the design and delivery of joined-up services that reflect what matters most to our local population.
