Work has been progressing to transform community mental health services that support people with severe mental illness in the Black Country.

From 11 to 15 September, we are hosting Community Transformation Week – an opportunity for you to discover our achievements so far when reshaping community mental health services that support people with severe mental illnesses.

What is the community mental health transformation?

The community mental health transformation is a national programme in the NHS Long Term Plan. This programme will ensure that the Trust works with primary care, local authorities, the voluntary sector and broader organisations to transform mental health care and support in the Black Country. The Trust aims to deliver care and support that is more accessible, flexible and tailored to suit an individual’s needs.

Throughout Community Transformation week, we will share information about the progress that we’ve made so far, along with a video showing how a service user’s journey will change, and why it’s so important to get involved in this work.
